The NREMT® AEMT cognitive exam is a linear computer-based test (not adaptive), administered at Pearson VUE testing centers or via OnVUE remote proctoring. Every candidate receives exactly 135 questions with a 3-hour time limit; the fixed-length linear format means all candidates complete the same question set. The exam includes clinical judgment scenarios and Technology Enhanced Items (TEIs) introduced to ALS exams in July 2024. Passing is based on a scaled score of 950 on a 100–1500 scale. Candidates must also meet the ALS Student Minimum Competency (SMC) requirement. The exam fee is $110 per attempt.
