About the NREMT® EMR Exam

The NREMT® EMR cognitive exam is a computer-adaptive test (CAT) administered at Pearson VUE testing centers or via OnVUE remote proctoring. The exam contains 90–110 total questions, of which 30 are unscored pretest items — leaving 60–80 scored questions. The time limit is 1 hour 45 minutes. The exam uses the 2025 BLS blueprint (effective April 7, 2025), organized into five assessment-flow domains with Primary Assessment carrying the highest weight at 37–41% of questions. Approximately 15% of clinical content involves pediatric patients, integrated throughout rather than grouped separately. The exam now includes Technology Enhanced Items (TEIs) — multiple-select, drag-and-drop, and ordering questions — in addition to traditional multiple-choice. Passing is based on a scaled score of 950 on a 100–1500 scale; the NREMT does not use a fixed percentage-correct passing threshold. The exam fee is $88 per attempt. Candidates must also meet a state-approved BLS skills competency requirement separate from the cognitive exam.
