About the NREMT® EMT Exam

The NREMT® EMT cognitive exam is a computer-adaptive test (CAT) administered at Pearson VUE testing centers or via OnVUE remote proctoring. The exam contains 70–120 total questions, of which 10 are unscored pretest items. The time limit is 2 hours regardless of how many adaptive questions a candidate receives. The exam uses the April 2025 BLS blueprint organized into five assessment-flow domains, with Primary Assessment carrying the highest weight at 37–41% of questions. The exam now includes Technology Enhanced Items (TEIs) in addition to traditional multiple-choice questions. Passing is based on a scaled score of 950 on a 100–1500 scale. The exam fee is $104 per attempt. Candidates must also pass a state-administered psychomotor (skills) exam in addition to the cognitive exam.

The EMT exam covers basic life support and assessment at the entry level. The Paramedic exam covers the full advanced life support scope of practice, including 12-lead ECG interpretation, advanced airway management, and a broad drug formulary. The Paramedic exam is a longer computer-adaptive test (110–150 questions, 3.5 hours) with a higher clinical judgment weighting.
