About the NREMT® Paramedic Exam

The NREMT® Paramedic cognitive exam is a computer-adaptive test (CAT) administered at Pearson VUE testing centers or via OnVUE remote proctoring. The exam contains 110–150 total questions, of which 20 are unscored pretest items. The time limit is 3.5 hours. The July 2024 ALS test plan places Clinical Judgment as the highest-weighted domain at 34–38% of questions. The exam includes clinical judgment scenarios, multiple-select items, drag-and-drop questions, and traditional multiple-choice — reflecting the Technology Enhanced Item (TEI) formats introduced to ALS exams in 2024. Passing is based on a scaled score of 950 on a 100–1500 scale. The exam fee is $125 per attempt. Candidates must also meet the Paramedic Student Minimum Competency (SMC) requirement.

The Paramedic exam covers the full ALS scope — advanced airway management, 12-lead ECG interpretation, complex pharmacology, and advanced cardiac emergencies — and uses a computer-adaptive format (110–150 questions). The AEMT exam uses a fixed 135-question linear format and covers a narrower intermediate ALS scope.
